What is a dental clinical note template?
A dental clinical note template is a pre-structured record for a specific procedure, with prompts for the elements a note is normally expected to capture: date and provider, reason for the visit, relevant history, clinical and radiographic findings, diagnosis, the consent discussion, anesthetic, the procedure with materials and technique, the patient's response, post-operative instructions given, and the plan for the next visit. Using a template makes notes more consistent and faster to write than starting from a blank field.
